How To Register a Claim:-

Death Claim will be registered on the date of receipt of the written intimation and the below mentioned documents to be submitted to Master Policy Holder

Please always quote your member id or Certificate of insurance for faster process of Claim

The following documents to be submitted:-
  • Filled-Insurance Claim form
  • Death Certificate of the Deceased Person
  • KYC of Deceased Person
  • KYC and Bank details of the Nominee
